Inflatable penile implants consist of two cylinders, a reservoir and a pump. Squeezing the pump moves saline solution from the reservoir into the cylinders to provide an erection. Deflating the cylinders transfers the fluid back to the reservoir and the penis becomes flaccid. In a 2-piece inflatable penile implant, the reservoir is located at the base of the cylinders while in a 3-piece implant the reservoir is a separate part that is implanted in the abdomen. Inflatable implants have the highest patient satisfaction rates and are less likely to erode compared to malleable implants. They provide superior rigidity and are a good choice for people who have a large penis. Inflatable implants are also easier to conceal in a flaccid state compared to malleable implants. However, because they have more parts, they have a higher rate of mechanical failure compared to malleable implants and therefore a greater need for revision surgery to replace the device.
A note on cylinders: A critical difference in penile prosthesis implantation in transgender men compared to cisgender men is the lack of the protective tunica albuginea to contain the implant in the phallus. This causes more movement of the implant under the skin and increases the risk of migration and erosion of the implant through the skin. For this reason, it is common to have only one of the cylinders implanted instead of two.
